Use this OSHA-compliant checklist to determine if all employees have been properly trained to work within confined spaces, site hazards have been identified and a rescue and safety plan is in place.
1. All personnel involved in the entry have been property trained
2. Training has been documented
3. Written program has been reviewed by staff
4. Monitoring equipment has been calibrated
5. The space has been tested to ensure oxygen is sufficient
6. The space has been tested for additional hazards
7. All physical hazards in the space have been identified
8. If ventilation is required the intake source is safe
9. If applicable, the permit contains all required info. (per OSHA) and has been signed by the entry supervisor
10. Necessary PPE and equipment is available
11. Rescue plan is in place
12. Rescue personnel are training in CPR
13. Space has been properly guarded or barricaded
14. Means of communication between entrant and attendant has been established
