Working In Enclosed Spaces Inspection

1. Have crew members and workers been trained in Enclosed Space Entry procedures?

2. Are vaults, manholes or enclosures labeled as Enclosed Spaces, Confined Spaces or Permit Required Confined spaces?

3. Does the company require that enclosed spaces are evaluated for any atmospheric pressure or temperature differences and by testing for a hazardous atmosphere prior to workers entering the space?

4. Does the space contain or have a potential to contain a hazardous atmosphere? (Examples: flammable mixtures, toxic gas or oxygen deficiency that may expose employees to the risk of death, incapacitation, or acute illness.)

5. Are Personnel involved with the entry knowledgeable of industrial or other discharges that may make this confined space unsafe due to dangerous air contaminants during entry?

6. Does the space contain a material that has the potential for engulfing an entrant? (Examples: liquids or granular solids.)

7. When working in an enclosed space, is an attendant trained in first aid, CPR, and rescue procedures stationed above the hole and maintaining communication with the worker(s)?

8. Are personnel using the atmosphere analyzer trained on its use and understand how to operate it?

9. If using an atmosphere analyzer, is the atmosphere analyzer’s calibration current, and has it passed field calibration (bump test)?

10. Are workers required to wear full-body harness attached to a hoisting device or similar apparatus for effecting a non-entry rescue?

11. Does the company provide Safe work practices and procedures for entry into, and work in, enclosed spaces and for rescue of employees from such spaces?

12. For manholes and permit required confined spaces; is rescue equipment (or a rescue service) immediately available and ready for the prompt rescue of an injured worker if needed?
