Utilize this inspection prior to each work shift before using a bucket truck to verify the equipment and all of its components are in safe operating condition. Can be used by foreman or general contractors.
1. Does the company require a thorough pre-trip vehicle inspection prior to using the equipment?

2. Does the inspection include: engine fluids, tires and wheels, signallling/warning lights and flashers, fire extinguisher and first aid kit, etc.?

3. Does the preoperational check of the areal device include: hydraulic reservoir level, visual inspection of boom, hydraulic cylindars, hoses, and fittings; insulating components?

4. Has the operator engaged the PTO and tested the outrigger interlock system prior to starting work?

5. Has the operator checked the worksite for: excessive slope, obstructions, soft or insufficient support for outriggers, overhead obstructions and electrical conductors?

6. Has the crew lead or Supervisor determined that the work site is free from: unauthorized persons, adjacent traffic, underground utility components or septic systems; and that weather conditions are favorable to conduct elevated work?

7. Have both the lower controls and upper (bucket) controls been operationally tested prior to starting work?

8. Have all hydraulic tools, fittings and hoses been inspected prior to being pressurized for use?
