Trench and Excavation Guarding Inspection

1. If the trench is at least 4ft deep, has a ladder been provided for access into trench?

2. Are ladders used in excavations secured?

3. Do ladders extend 3 feet above the edge of the trench?

4. If the trench is over 5ft deep, is trench adequately shored or braced to prevent trench collapse?

5. Has the trench been inspected by a competent person prior to any entry?

6. Is guarding in place to prevent unintentional entry?

7. Is material at least 2' away from the edge of excavation?

8. Is guarding in place to prevent dangers from the falling material or objects?

9. Is guarding in place to prevent water from rushing into the excavation?

10. Are walkways and bridges over excavations equipped with guardrails and toe boards?

11. Is there a warning system in place for pedestrian and vehicular traffic? (e.g. Barricades or stop logs, flashing lights at night.)
