Telehandler/Rough Terrain (RT) Forklift Work Area Audit

1. Is the operator qualified to operate the Rough Terrain Forklift?

2. Are there any holes or soft ground in the area where the forklift will be operating?

3. Has the area been checked for the location rebar stakes that could be sticking up and would create a hazard?

4. Is the operator aware of the uneven terrain to be traversed?

5. Is a spotter available to assist the operator as needed?

6. Is the terrain and surface area in stable condition

7. Will any part of the operation be on a slope or incline/decline?

8. Is the area blocked off to prevent unauthorized personnel?

9. Are the workers aware of any overhead wires that could create a hazard?

10. Are there any overhead obstructions? (ceilings, overhead doors, platforms)

11. Is there any equipment overhead that might cause a struck-by injury? (fans, lights, etc.)
