2. Cross Braces in Place and Secure?

3. All Bolts, Pins, and Clips Secure?

4. No Visible Damage or Deterioration (e.g., rust, cracks)?

1. Platforms Fully Decked with No Gaps?

2. Platform Boards Properly Supported and Secure?

3. Guardrails (Top and Mid) & Toeboards Installed and Secure?

4. Access Ladders/Stairways in Good Condition?

1. Fall Protection Equipment Available and in Use? (REQUIRED PHOTO OF FALL PROTECTION IN USE)

2. Fall Protection Harnesses stored properly on all sites with scaffolding in use? (PHOTO REQUIRED OF FALL PROTECTION)

3. Employees properly trained on use of fall protection harnesses by competent person? (COMMENT NAME OF COMPETENT PERSON)
