2. D-Rings and Connectors Free of Deformation or Cracks?

3. Harnesses Properly Worn and Adjusted by Workers? (REQUIRED PHOTO OF PFAS PROPERLY IN USE)

4. PFAS Anchor Points Identified and Secure? (REQUIRED PHOTO OF ANCHOR POINTS)

5. Lifelines (Vertical/Horizontal) Properly Installed and Inspected

6. PFAS Worn by employees walking on top plates, stepping off ladders or above 4' from floor on surfaces with leading edges or unprotected openings.

1. Guardrails Installed at Leading Edges? (REQUIRED PHOTO OF GUARDRAILS INSTALLED)

2. Top Rails Installed at Correct Height (42 inches and 3 inches)?

3. Guardrails Free of Damage, Gaps, or Missing Sections

4. Warning Lines Installed Where Guardrails Are Not Feasible (REQUIRED PHOTO IF GUARDRAILS NOT FEASIBLE)

1. Openings Properly Covered or Guarded (REQUIRED PHOTO OF OPENINGS GUARDED)

2. Temporary Guardrails Installed Around Roof Edges (REQUIRED PHOTO OF ROOF EDGES)
