Use this inspection to identify and slip, trip or fall hazards in the workplace, such as cords or hoses in a walkway area, standing water or unguarded areas where a worker could fall farther than 4 ft.
1. Are aisles and passageways properly illuminated?

3. Is the site cleaned-up at regular intervals and at end of shift?

4. Are openings greater than 2" in floor surfaces covered and marked?

5. Are wires and cables from the ceiling hanging less than 7ft?

6. Is standing water or other liquid present on walking an working surfaces?

7. Are cables, cords and hoses that cross walkways enclosed in crush-resistant cable protector or rigid conduit?

8. Is there a drip cloth or barrier under all fuel driven or hydraulic equipment when it is positioned or stored in the same place for a period of time?

9. Is the ground area six feet around structures under construction level and free from holes, protrusions and irregularities?

10. Is piled or stored material stacked in a stable manner so it's not likely to fall over?

11. Are all areas where workers could be exposed to falls greater then four feet (4 Ft.) guarded by handrails or barriers greater than 39" high?

12. Are temporary stairways and ladders checked daily for damage, irregularly spaced steps/rungs, secure handrails and adequate lighting?
