Heavy Equipment Safety Underground Checklist

1. Are vehicles stocked with appropriate emergency supplies (flares, cones, flashlights, fire extinguisher, first-aid kit, etc.)?

2. Are all vehicles properly maintained and routinely inspected and serviced?

3. Are procedures in place to review any vehicle modifications before they are made (e.g., adding trailer hitches, accessibility aids, changing tire/rim size, or window tinting)?

4. Are employees instructed in safe driving behaviors, both before they are allowed to operate vehicles for work purposes, and on an on-going basis?

5. Do employees getting in and out of equipment use three points of contact and available steps and ladders vice jumping to the ground?

6. Have collision avoidance/warning technologies been installed and properly maintained on mobile equipment?

7. Is substantial overhead protective equipment provided on high lift rider equipment?

8. Are all drivers trained on the importance of using flags or strobe lights on the cabs of their vehicles to make haulage truck operators aware of their location?

9. Are operators and other employees trained not to drive or park smaller vehicles in mobile equipment's potential path of movement?

10. During loading and unloading of materials from material handling equipment, vehicles are parked on level ground with brakes set?

11. Workers are wearing proper foot, hand, head, eye and high visibility PPE per employer instructions?

12. Are operators required to communicate and verify any planned movements and location upon entering or exiting a work area?

13. Are operators and other employees trained on the limited visibility and blind spot areas that are inherent to the operation of large equipment?
