Hazard Assessment Template

1. Are work areas kept clean and orderly?

2. Has a regular inspection schedule been set up to survey work area conditions and degree of employee exposure to risk?

3. Does type of PPE used match the needs of current operations?

4. Is proper PPE provided, worn, and maintained properly according to hazards present?

5. Is the compressed air for cleaning under 30 psi or fitted with a low pressure nozzle?

6. Are adequate engineering and protective steps taken to protect workers from chemical exposure hazards (e.g., handling of chemicals during production, or exposure from spills/leaks)?

7. Are all hazardous chemicals appropriately labeled per HAZCOM requirements?

8. Is eating, drinking, chewing gum, smoking, application of makeup or storage of food prohibited in the production area?

9. Are odors, dusts, and emissions from machining, welding, painting, curing, and other contaminant-producing manufacturing or production activities isolated or removed?

10. Has noise monitoring been conducted for new operations and when there have been changes to the facility?

11. Are storage areas kept free from accumulation of materials that constitute hazards from tipping, tripping, fire, explosion or pest harborage?

12. Are operating procedures readily available to employees who work on or maintain a hazardous process or equipment?

13. Have all employees been adequately informed about the work in the production area, its risks, and what to do if an accident occurs?

14. Have all employees been adequately informed about how to report suspected hazards in production area, and are Supervisors aware of their responsibnility to correct or mitigate hazards?
