Falls in Construction Checklist

1. Are all areas where workers could be exposed to falls greater then four feet (4 Ft.) guarded by handrails or barriers greater than 39" high?

2. Are ladders inspected before use and taken out of service if damaged?

3. Are employees using the ladders observed maintaining 3-points of contact/control while climbing the ladder?

4. Are workers who access roofs trained to not approach an unprotected roof edge without personal fall protection equipment?

5. Are workers who use personal fall protection equipment (I.e.Harnesses, Lanyards,SRLs, etc.) properly trained in inspection and use of the equipment?

6. Are all personnel working at heights using Fall Protection?

7. Are narrow base scaffolds (I.e. rolling scaffolds) inspected before each use and maintained in good operating condition?

8. Are workers on rolling scaffolds observed to lock scaffold wheels to prevent movement while in use?

9. Are workers on scaffolds observed to remain inside guard rails and not climbing on guardrails while working?

10. Are workers on rolling scaffolds observed to not move the scaffold while on the platform (I.e. "scaffold surfing") while in use?

12. Is all personnel working at heights on scaffolding that may step off the scaffolding using Fall Protection?
