Fall Arrest System Inspection for Roofing

1. Is the identification tag present?

2. Is the safe working load (SWL) present on the tag?

3. Is there any damage to the webbing or straps of the harness? (fraying, rips, cuts, abrasions, burns, ect.)

4. Is the D-ring and all metal connectors undamaged and free of rust or corrosion?

5. Are all buckles secure and undamaged?

1. Does the lock and gate function correctly?

2. Are hooks free of damage, rust, and/or corrosion?

1. Is the carabiner free of cracks, damage, or other deformation?

2. Is the self-locking mechanism in good working order?

1. Is the housing free from damage, cracks, and/or corrosion?

2. Are all labels in place?

3. Does the lock and gate function correctly?

4. Are hooks free of damage, rust, and/or corrosion?

5. Is the impact indicator NOT showing? If the impact indicator IS showing, remove it from service.

6. Pull the entire length of the line. Are there any excessive wear, fraying, cuts, kinks, etc?

7. When retracting the line, does it retract smoothly?

8. Quickly pull the line. Does the unit lock correctly?

1. Is there an appropriate anchor point to tie off to?
