Construction and Demolition Health Hazard Checklist

1. Has a qualified person confirmed that no asbestos or asbestos containing materials be disturbed ?

2. Will any lead containing materials be disturbed?

3. Will there be demolition of concrete or silica containing materials?

4. Will there be welding, cutting, brazing or soldering?

5. If welding creates hazardous air emissions, has the welding area atmosphere been sampled, and is installed exhaust ventilation adequate?

6. Will gasoline powered tools or fuel burning generators or compressors used at the work site?

7. Will there be chemical stripping, etching or paint removers used at the job site?

8. Will solvent based or epoxy based coatings or paints be used at the job site?

9. Will there be high noise levels ( >90 Db) in areas where people are working for up to 8 hours at a time?

10. Is approved hearing protective equipment available to every employee working in noisy (where noise levels exceed 80 dBA) areas?

11. Will workers be exposed to flammable fumes from soil or storage tanks or drums?

12. Will workers be exposed to biological hazards such as insects, snakes, sewage, decaying animal or vegetable materials at the job site?

13. Will workers be exposed to lasers, X-Rays, high levels of UV light, RF or ionizing radiation on the job site?

14. Will workers be exposed to contaminated soils containing heavy metals, PCBs, cholrinated solvents or petroleum products on this job site?
